Services offered by Care Connect
To ensure that you receive exceptional care, our team has undergone comprehensive training in functional and integrative healthcare. Without Training: We offer services to support individuals who live in their own home or family home in the community living service categories listed in the covered services section.
With Training: We provide support and training services to individuals who live in their own home or family home.
With Family Training: We offer training services to both individuals and their family members who live in the family home.
We provide overnight assistance and supervision to individuals who require help in one of the covered service areas. Night supervision services include both asleep and awake staff in the person’s own home.
We offer community living services to adults aged 18 and older who live in a living unit of a provider-controlled ICS setting. Our ICS services cover training and support to meet individualized assessed needs and goals in community participation, health, safety and wellness, household management, and adaptive skills. ICS services can be provided up to 24 hours per day in the person’s living unit or in the community.
We assist individuals in exploring competitive, integrated employment opportunities in their community. Our exploration activities and experiences aim to enhance a person’s knowledge, interests, and preferences to make informed decisions about competitive employment.
Employment Support Services: We provide personalized support to help individuals maintain paid employment in community businesses or settings. Our employment support services include both job support and self-employment/microenterprise support services.
We offer tailored services to help individuals achieve competitive, integrated employment, become self-employed, or establish a microenterprise business in their community. Our services include job development and self-employment/microenterprise development services.
Our 24-hour emergency support service is designed to offer reassurance and quick response during critical times. We have established protocols and procedures in place to handle emergencies efficiently and effectively, ensuring the well-being and safety of our clients.
Our trained and compassionate staff members are dedicated to creating a warm and welcoming atmosphere for our clients. We offer a range of engaging activities and programs designed to promote cognitive stimulation, physical well-being, and social interaction.
At Care Connect, we offer Independent Living Services (ILS) to support individuals in maintaining their independence and living fulfilling lives in the comfort of their own homes. Our ILS program is designed to provide personalized assistance and empower clients to actively participate in their daily activities and make choices that enhance their quality of life.

Caregiving is a demanding and stressful job, especially for family members who are caring for a loved one full-time. At Care Connect Home Health, we offer respite care services, providing temporary relief for family caregivers so that they can take a break, run errands, or attend to their own needs.
